Case Manager
We are seeking a Case Manager who has experience working with the homeless population in the community. We are looking for a Case Manager to enhance the quality of client management, promote the program in a positive manner and work directly with the clients to improve their quality of life while participating in the program. The successful candidate will be accountable for supporting, advising and helping the residents obtain supportive services to help them move to complete self-sufficiency and graduate the program.
Responsibilities
Complete monthly stats
Complete ESG and TANF paperwork
Assist with obtaining vital records and documents
Track vital records needed and document outcomes
Review Resident handbook with each family
Assist residents with filling out supportive services paper work
Schedule weekly meetings with residents and document
Verify residents’ employment or verify job searching and document
Monitor and update files on each resident
Facilitate weekly residents’ meeting
Review with resident’s weekly schedule and calendar events such job search, W2L, training classes, and etc.
Refer residents to In-House Services based upon need
Attend Treatment Team meetings and report on residents progress/goals
Attend all scheduled meetings and training
Advise and coordinate resident activities
Adhere to professional standards as outlined by protocols, rules and regulations

Education
Prefer a Bachelor’s degree in Human or Social Services in lieu of degree work experience will be considered
Training which provides the knowledge, skills and abilities necessary to perform the job duties
